Becoming a Client

Effective treatments for many conditions are available for adults, adolescents, and children at the Anxiety Disorders Center at The Institute of Living.

The first step in making an appointment is completing our online New Patient Form. A clinician will review this form to help determine whether our center is a good fit for your current treatment needs. If we have any questions about your responses on the New Patient Form, a clinician will call you to ask follow-up questions. If it is determined that the Anxiety Disorders Center is not the best place for you to receive treatment, you will be provided with outside referrals. If it's determined that our center may be a good fit for your current treatment needs, you will be placed on our waitlist for an initial consultation appointment.

We will call you at the phone number provided on the New Patient Form to schedule an initial intake appointment. When we schedule the appointment with you, we will send you the required forms and questionnaires that need to be completed at least one week prior to your scheduled initial appointment. The clinician meeting with you during your intake appointment will review your questionnaires prior to your appointment so that they can prepare for and make most effective use of your first meeting.

At the intake appointment, you will meet with one or two clinicians for 2-3 hours. During this meeting, the clinician(s) will ask many questions, including questions about your current concerns, your treatment history, and current medications. The purpose of this meeting is to understand your concerns and your relevant history so that we can make good treatment recommendations. Treatment recommendations may include participating in treatment at our Center, or a referral to a program that is a better fit for your current concerns.
