The Perinatal Day Program accepts referrals from providers or self-referrals from clients.
The Perinatal Day Program Referral Guidelines Admission criteria:
To be eligible for this program, clients must be at least 18 years of age, either pregnant or within 18 months of giving birth. They must have been newly diagnosed with a Perinatal Mood and Anxiety Disorder (PMAD) such as postpartum depression, anxiety, OCD or have worsening mental health concerns during or after pregnancy. We treat clients with a history of perinatal loss who are currently pregnant.
All clients will have a preliminary intake with a perinatal therapist or reproductive psychiatrist to determine whether the Perinatal Day Program is the best treatment option based on each individual client’s clinical symptoms and needs.
Provider Referral Form
For those providers referring from within the Hartford Health Care System, please submit a referral via EPIC: Submit an order to AMB REFERRAL TO: Adult Behavioral Health and select ‘HH IOL INST PSY PERI'. Please indicate if the referral is for OP, IOP, or Consultative Services.
For those providers referring from the community, please submit a provider referral form and have the client call the Institute of Living’s Access Center at 860.545.7200 to schedule an initial clinical evaluation.
